Youth Volunteer Waiver

  • I the undersigned, the parent or legal guardian of the child or children listed below (collectively the “child”) elect to donate my and/or my child’s time, effort and services to assist Random Acts of Flowers in pursuit of its mission. I represent and warrant that: I am the parent or legal guardian of the child with the authority to executive this waiver on behalf of myself and the child; and that the child is over the age of 12 years old.

    I recognize, understand and voluntarily assume, on behalf of myself and the child, the inherent risks associated with volunteering at Random Acts of Flowers, including but not limited to the possibility of any injury to person and/or property which I and/or my child may suffer, or others many suffer.. I acknowledge and agree that the child must, and shall be, under my supervision or the supervision of their group leader at all times while volunteering at Random Acts of Flowers and the child is not to be left alone or unattended.

    I understand that Random Acts of Flowers desires to accept the donation of my and/or my child’s time, effort and services subject to the terms and conditions contained in this Waiver of Liability.

    In consideration of the mutual promises of the parties contained in this Waiver, or of the acts to be performed by either, I hereby knowingly and voluntarily release and waive, and agree to indemnify and hold harmless Random Acts of Flowers on behalf of myself and the child, any and all claims, damages or causes of action which I or the child may have or incur against Random Acts of Flowers, its officers, directors, agents, affiliates, employees and other volunteers as a result of my and/or my child’s donation of time and effort to Random Acts of Flowers. I also authorize Random Acts of Flowers to take my or my child’s picture while volunteering and use in Random Acts of Flowers publications.

    Parents, guardians and child are expected to exhibit appropriate behavior in their dealings with others and be cognizant that their personal behavior may reflect on Random Acts of Flowers. Inappropriate behavior includes any activity through which a parent, guardian or child reduces or destroys his or her effectiveness, the effectiveness of fellow volunteer’s, or the ability of Random Acts of Flowers to fulfill its mission. This includes but is not limited to engaging in antisocial, discriminatory, or illegal activities or demonstrating inappropriate behavior.

    This Waiver shall be binding upon the parties hereto, their heirs, assigns and legal representatives.

  • Youth Volunteers Require Adult Supervision. If volunteering as a individual, please include the name of the adult supervisor volunteering with you. If you are volunteering with a group (i.e. boyscouts) please include the name of the Group Leader.
